I/We, the parent(s)/guardian(s) of the child listed below, understand that by checking the box below am granting permission for our child to participate in Calvary Chapel of Yelm Vacation Bible School. I/We understand that we assume all risks and hazards related to incidental participation, including games and activities related to Calvary Chapel of Yelm Vacation Bible School. I/we waive the release and absolve any indemnity and agree to hold harmless any and all volunteers of Calvary Chapel of Yelm Vacation Bible School and Calvary Chapel of Yelm Church for any claim arising from any injury (or previous injury) or damage to our child or his property, or any other reason.
